Insurance Plans and Payment
We accept the majority of PPO insurance plans and we are not part of any HMO plans. It is the patients responsibility, however, to verify insurance eligibility. We will bill your insurance company directly. It is our office policy that co-payments be paid at the time of your appointment.
Appointments for physical exams and for sick visits are scheduled during business hours Monday through Friday. Saturday appointments are also available about once every three weekends. Our staff will try to make reminder calls 24 hours prior to your scheduled appointment. If you are unable to keep an appointment, please notify our office at least 24 hours prior to your appointment. Same day cancellations or “no shows” are subject to a $50 cancellation fee.
Family Administrative Fee
To provide timely, efficient and high-quality care, Oak Park Pediatrics has instituted an annual non-contractual administrative fee. This fee is due annually on the 1st of the year. This fee covers specific services we provide that are not covered by insurance companies. When a family joins our practice throughout the year, this fee will be "prorated". This annual fee is in lieu of individual charges for the services listed below.
- Access to Board-Certified Pediatricians on-call after hours
- Same-day appointments for sick visits
- In-office laboratory services
- Recommended and coordinated care with other pediatric specialists and therapists
- Call in and fax prescriptions, both new and refills
- Write letters to schools and airlines for your child’s needs
- Write appeals, letters of medical necessity and pre-authorization referrals to insurance companies to advocate for the needs of your child
- Send copies of medical records and reports to specialists
- Complete health forms for school/camp/sports in a timely manner
- Submit insurance claims on behalf of all patients that are in network
The annual family administration fee for Oak Park Pediatrics is:
- $150.00 per year for families with a single child
- $200.00 per year for families with two children
- $250.00 per year for families with three or more children
“Well” Entrance and “Sick” Entrance
Our office has separate entrances for those who are healthy, ill and for those who may have a contagious rash. We ask that all noncontagious patients enter through our “well entrance”, and those who may be contagious to enter through our “sick entrance”.
If you are expecting, you are welcome to schedule a visit to meet with the Doctors. These are scheduled at the end of the day, on Monday, around 5:15 PM, and lasts about 10-15 minutes. There is no charge for these visits.
School and Camp Forms
Please allow up to 48 hours for the completion of school and camp forms and immunization records. Your child must be current with his or her physical exam for the forms to be completed. Completed forms can be picked up in the office or can be mailed directly to you if we are provided with a self-addressed stamped envelope. We cannot fax or mail forms to third parties (e.g. schools or camps).
For pharmacy refills, please have your pharmacy fax the request during office hours. Our fax number is (818) 707-2430. For controlled substances (ADHD medication), please allow at least 3 days for your medication to be refilled. These medications cannot be called in to your pharmacy and must be picked up in person.
Calling us with Questions
If you have a routine question about your child, please call the office during regular office hours. We ask that you call after hours only for urgent matters. During regular office hours, your question may be answered by one of our nurses. If you prefer, a message can also be left with your doctor. Our doctors will return your call between patients, during lunch hour, or after 5 pm.
When the office is closed, there is always a pediatrician on call. This will be one of our doctors or a pediatrician in the community. In order to reach the doctor on call, please dial our office number and you will be connected to our answering service. The answering service will then notify the doctor.
Dr. Matthew, Dr. Hochman and Dr. Tenenblatt have privileges at Los Robles Hospital. Dr. Hochman also has privileges at Cedar Sinai Hospital. If you are delivering at Los Robles hospital, one of the doctors will likely be able to take care of your newborn after delivery. If you are delivering at Cedar Sinai hospital, and you would like doctor Hochman to be there, there is a $100 service charge per day to care for your newborn.
